Select your insurance plan below for plan-specific information. An insurance company may offer PPO, HMO, DMO, employer, union or managed-care products with different networks. Participation with one product does not automatically mean participation with every product.

New York State advises patients to confirm participation for the exact plan and location before receiving services. Search the official NYS Provider & Health Plan Look-Up ↗

Before your appointment

  1. Find the Member Services number on your insurance card.
  2. Confirm that coverage is active.
  3. Ask whether Dr. Oleg Lyalin / Hollywood Smile NY participates in your exact network.
  4. If your plan requires an assigned dentist, confirm that the assignment is active for our office.
  5. Ask about deductibles, copayments, coinsurance, annual maximums and authorization requirements.

Bring your insurance card, photo ID, member or subscriber ID, the primary insured person’s information and details of any second plan. Two cards from the same insurer can still have different benefits.

Managed-care and Medicaid questions

HMO, DMO and managed-care plans may require you to select or be assigned to a participating dentist. Ask Member Services: “Am I required to select or be assigned to a dentist before my appointment?” New York Medicaid materials explain that covered services and networks vary by program and plan.

Self-pay patients are welcome

You do not need dental insurance to become a patient. Before treatment, ask about examination costs, X-rays, treatment options, estimated patient costs and alternatives. The office can help collect information, but the insurer determines benefits under the patient’s contract. Verification is not a guarantee of payment.

Questions patients ask

Does the insurer name alone confirm coverage?

No. Verify the exact product, network, location and procedure.

Can I use two dental plans?

Tell the office about both policies and bring both cards. The insurers determine which plan processes a claim first and how benefits coordinate.
